It works as one big washer: the fittings pass through the plate and the panel, the nuts pull the plate down flat, and the load spreads across the whole disc instead of five small washers crushing into carpet. Deburr the panel, and if you're going through carpet, cut it back to the plate's edge rather than crushing it under the disc. Put the plate on the finished side and the nuts on the back, and torque the nuts evenly. The fittings are the seal, the plate is not; you don't need sealant between the plate and the panel unless you want it watertight, in which case a bead of RTV under the disc is fine.
